H.E.R.O. is a nationally accredited diabetes program that has successfully worked with Senior Centers, Independent living facilities, corporate employers, governments and faith based communities to help people manage their diabetes. HERO, Inc. (Healthcare Education Resources and Outreach) offers comprehensive diabetes education and testing, proactive consultation and planning, and health risk assessments in an easy-access manner. HERO: • Is the only program to have earned national accreditation from the American Association of Diabetes Educators (AADE).• Offers interventions to high-risk participants that can reduce the incidence of poor outcomes.• Helps participants develop behavioral changes that promote healthy lifestyles. • Incorporates both performance measures and reporting - the reporting component allows us to deliver updates to participating healthcare providers for appropriate follow up.
