Using a system of whole person assessment, it encourages services to integrate around an individual to improve outcomes.Its products include the Risk Tracker system to help organisations who provide preventative services to record their work and demonstrate success, and the Referral Tool, built to complement any data system and supporting health and care professionals to make effective referrals.
